You use "how do you..." when you're talking to someone directly (e.g. to a friend, "how do you do that?!"). I usually use "how to..." when I'm searching for something online trying to find out how to do something (e.g. searching in google: "how to dunk").

Use 'how to' if you say any question for yourself. and use 'how do you' if you give any question for anyone (person). i think that's all.

How do you is a simple sentence. how to, is it a clause or a phrase?
