what does "five miles up"mean? 【There, five miles up in the sky, his polished shoes resting on a blue rug adorned with a golden eagle and thirteen gold stars, President Kennedy could hold conferences and telephone subordinates as easily as though he were in his Castle's West Wing.】 what does "five miles up"mean? the plane is five miles up in the sky? or the president's shoes are five miles up in the sky? I think both explainations make no sense.

Yes. This must mean that President Kennedy could hold conferences in the sky (5 miles above ground) as well as he could have done in his castle. He is in the plane and his shoes are resting on a blue rug inside the plane.
