I would be offended if you didn't come by. and I would be offended if you hadn't come by.

Both are correct - almost.
'I would be offended if you hadn't come by' would need some sort of time clause.
'I would be offended if you hadn't come by before I left' or 'I would be offended if you hadn't come by during the day'
