As Tumpliner said, in your example 'then' and 'and then' have exactly the same meaning. However, we would be more likely to use 'before' in your sentence. These sentences sound natural: 'It is best if you eat breakfast before you go to work' (general advice) or 'You had better finish your breakfast before you go to work' (one time advice or instruction)

"then" is a reference to a point of time. You can start sentence with "Then", but not with "and then". You can never start a sentence with "and". Otherwise, the meaning is the same.
