How would you explain this sentence in the context? 24 June St. John’s Day, all the church bells ringing on a Tuesday. The maid says it’s a signal for the lepers to bathe. Today is the only day of the year they are allowed to touch water. No wonder they smell as they do. How would you explain the last sentence: they smell as they do? Thanks!

In this context, "as they do" can mean "like they do" or "the way they do". It's no wonder that they smell a certain way - that is, bad - because they haven't been allowed to bathe.
