It means that the two things are equal. To refrain from an act = to not do something, is no less of an act = is not any different than doing something, and to commit = to do an act. If you put it all together, it comes down to each choice holding equal weight. It's similar to saying, "there are always three choices: to say yes, to say no, and to abstain."
