Do "and what not" and "and the like" mean the same? Is either okay to use in the following? e.g. Inside the box was full of gifts and letters and what not/and the like. My gut feeling is that "and what not" is British English and "and the like" American English.

They mean similar things. "And what not" is very informal, probably used more in speaking; "and the like" is still quite informal, but better for writing.
