Context would be useful. It's a phrase that would probably refer to experiencing something for yourself, rather than hearing or reading about it. You would usually just say 'first hand' rather than 'at first hand'. 'I saw the monster firsthand. It was fifty stories tall and looked like Godzilla crossed with an anglerfish.'
