Is my sentence correct? Forward to you the form which is duly signed and for your further action.

I do not think so. I'm not sure what exactly you're trying to say, but if you're using the imperative tense and telling someone to forward a form, it would make more sense, "Forward the form that has been duly signed for further action." Or if you're saying that *you* are forwarding something, it would be, "I will forward to you the form that has been duly signed for further action." Again, though, I'm not entirely sure what you're trying to say.
