Do you ever .. or Have you ever.. Please anyone help me with these two questions: Do you ever mix them up? vs. Have you ever mixed them up? What is the difference between those two sentences? Many thanks in advance?

The difference would be what you do currently. 'Have you ever' refers specifically to the past, so therefore implies that you perhaps have learnt the difference, and therefore would not make the mistake any more. 'Do you ever' is asking if you did mix them up, _and_ are likely to continue to do so.
