"Mr + name + surname" or "Mr + surname"? In formal English is it possible to write "Mr + name + surname" or is it better to write "Mr + surname"? Thank you very much!

The other option would be for emphasis of the first name.For example if you were afraid there might be confusion with another person sharing the same surname.

I think it depends mainly on formality. For example, if someone was being introduced on television, they'd use their full name. If you were being called for in a doctor's office, they'd just use your surname.
