It means you are thinking about someone. Perhaps they have been ill or someone they love has died, so you can offer compassion to them by saying ' you have been in my thoughts ' or 'I've been thinking of you ' ( only if it is true of course )

It is an expression that means you are thinking of someone. People will say this to someone when something tragic has happened to that person. For example, if my friend's grandmother died I would tell her "you are in my thoughts."
