There is no difference. They both mean the same thing. Another way of saying the first phrase would be to say "What do you like to do when you have free time'.

Nothing, basically. Out of personal preference I would use the first one: What do you like to do during your free time?

Nothing. "Free" and "spare" are the same. You can use either of them.
