Does "she still retains her facilities" mean "she is still healthy"?
Context:
she is 100 years old. Though she is blind and paralyzed in all but her right arm... she still retains her facilities to an unparalleled degree... she converses freely and...

It is more referring to mental health. For example she remembers people, places and events and isn't confused easily.

I don’t know what grammar guide you’re referring to, but “almost” is incorrect in this context.
You would never say “paralyzed in almost her right arm”.
If you wanted to say "almost paralyzed in her right arm." it would have been
"all but paralyzed in her right arm"
