It is more referring to mental health. For example she remembers people, places and events and isn't confused easily.

I don’t know what grammar guide you’re referring to, but “almost” is incorrect in this context. You would never say “paralyzed in almost her right arm”. If you wanted to say "almost paralyzed in her right arm." it would have been "all but paralyzed in her right arm"
