It looks like an email, so the format can be corrected. In terms of grammatical errors, I would say, " which we talked about LAST TIME, would it be possible for us to meet YOU to discuss ABOUT it on Sunday AT 8.00 AM/PM" The wording is not bad but to make it more accurate I would change the structure of the sentences.
