'Everything but the last line,' I said. 'The book, such as it is, is dedicated to you, and I want you to put down the last bit.' Question: Does ‘This custom began, as I say, after the second book’ mean ‘This custom began FROM THE THIRD BOOK?’ If not, what does it mean?

Yes it appears that the custom began with the third book.
