Those you've mentioned are all correct. There are other phrases to be used like: -what's the matter? -what's going on? Are you all right? Is everything ok?

@Nada "What the hell is going on?" is not the same as the others. People say that when they're angry rather than worried. It is quite a rude thing to say, and British people use it, too. I'd say context has some importance here, but other options would be "Is everything okay?", "Is there any problem?" or "Are you alright?".
