Is this means that 'He isn't noisy' or 'He is noisy'???? Please help me with this. Thank you in advance.

You can think of it this way. If he doesn't do noises quieter than screaming, that must mean he only does noises as loud or louder than screaming. So he is loud/noisy. It is certainly an interesting sentence.
