Fill in a Form If I sent a form and asking my customer to fill it out. But she sent the same form back without fill the form. Can I say "please resend the form, it seems like you attached the origing form" ? Is it ok to call it "origin form" or "blank form"?

I would say: “Please send back the completed form. It looks as though you mistakenly sent back the blank form. Thank you.”

"Original" form is correct. You may say this several different ways: Please send the completed form. Please fill out the form. Please complete the form.
