If anyone asks 'how do you do?' what would be the reply? Does it mean 'how are you?'

Ack, that should have been and answer, not a comment. "How do you do?" shouldn't be taken literally. It's a greeting used when you first meet someone, and the normal response would be to repeat "How do you do?" back to them.

Yes , they are interchangeable and mean exactly the same: In both cases " How are you ?" , " How do you do?" you can answer the same" " i am fine , thank you " .and you can add whatever you feel like saying after !
