1. Modern technology makes all our lives easier
( Is it ok if I replace " technology" with " equipment" ?)
2. For the number (1) and (6), why can't I use form to be able to in those sentences?

The use of "can" and "able to" is interesting. They are both about the *ability* to do something.
"Can" is a modal verb, so it's used with other verbs. For example: "can play" and "can't drive".
However "able" in the examples above is an adjective. Look at the no.4. Because the text already shows the "has", that means there is verb already there. The answer isn't "Amy has can ride a bicycle". Therefore to show the *ability* of Amy, we use "able to". You got that correct for that reason.
You might wonder why you got no.1 wrong. It's not totally inaccurate to write "she is able to play very well", it's just not very natural. It's more simple to just say "she can play very well". It's meaning about Amy's ability is very clear when using "can".

1. It really depends on what you intend to say, and in what context you are saying it. If you say "modern technology", you are referring to more than just the equipment related to technology. And if you were to say "modern equipment" without much context before that, it will not be very clear, because as a listener, we don't know what category of equipment you are talking about.
2. I think this exercise is only asking for either be / been able to as the answer.
for 1 and 6, if you use "is able to", you will change the meaning of the sentence slightly.
For example, no.6 , I would use "can't" because she is not allowed to drive. If you use "isn't able", then I think, you lose the idea of not being allowed to drive.

wants to be able to use, but it sounds as if she can't, physically can't, rather than wanting permission.
