The expression is expressing a great passion for something. It means to want something, but to want it with a real passion. It can be used in a romantic sense. For example, the expresson "Tengo ganas de ti." Basically meaning "I really want you." Tengo muchas ganas de visitar Londres = I really want to go to London.

It is to express a hard desire or craving for something. It has to be used with another complementing phrase such as "Tengo muchas ganas de dormir" - I really want to sleep. "Tengo muchas ganas" alone would hardly make any sense, only in case some activity or thing has been described before thus is being referred to. An example would be: "Quisieras conocer a algún famoso? - Tengo muchas ganas!" translated as: "Would you like to meet any celebrity? - I really want to"
Hope it helps
