O.K first choiced is not a word, it does not exist at all! You can say things like, which one should I choose, which one should be chosen, which one should be used, which one should I pick, which is correct, anything but choiced! The correct answer is all of them are ok depending on the context. tense of the passage, your preferred writing style, what is was being written for etc etc.

It is high time that this controversy____put an end to. A was B is C were D be It is high time that this controversy "was" put an end to. I guess the detailed explanation I can give is that: "was" is the correct choice because besides sounding better than the other choices, the controversy has already been there for sometime (in the past) and that it has already been resolved before the speaker even knew about its resolution. Let's say that the controversy ended yesterday and that the speaker knew that it had ended only today.. The speaker is talking in the present time (so he used "is high time") and the controversy was ended in the past (thus, "was put an end to").. This is by far the simplest example I can think of.. :)

Hmmm well, the problem is that the controversy hasn't ended yet. So I can't accept "was". You use the phrase "it's high time that..." to express a wish as a declaration, which we call a "mandative statement". Which makes me suspect it's subjunctive: "It is high time that this controversy BE put an end to." Sounds quite formal, doesn't it? :) Save 'is' and 'was' for facts.
