yes there is a difference. "What do you mean" is what you would say if somebody said something and you don't quite understand it. You are not sure you understand what they are trying to convey to you. "How do you mean" would be used much less often and the word "how" implies you want an explanation of how to do something, or in what way is the sentence you don't quite understand true or relevant.
