To say Hello, my name is (____).
こんにちは。わたし(if you're a girl) ぼく (if you're a guy) のなまえは(____)です。
konnichwa. watashi/boku no namae wa (____) desu.
hello. my name is (______)
This is formal speech. If you want to use very formal speech,
初めまして。(____)と申します。
hajimemashite. (your full name) to moushimasu.
nice to meet you. my name is (______)

Usually, you introduce yourself starting with your name. You would say, "Watashi wa (your name) desu, douzo yoroshiku" or わたしは____です、どうぞよろしく in Hiragana. This translates to, "I am (your name), nice to meet you". If you want to ask for someone's name, you simply say, "Onamae wa?" or おなまえは?(what's your name?).
