だれがあなた(に/の)名前を付けたの?
Dare ga anata ni/no namae wo tsuketa no?
polite:
だれがあなた(に/の)名前を付けたのですか?
Dare ga anata ni/no namae wo tsuketa no desuka?
に/の,Both OK.
The verb 'つける(付ける)' has many usages like 'put' or 'Put on'.
付ける:Tsukeru(v)
付けた:Tsuketa(v.p)
パンにバターを付ける (spread) butter on bread.
むねになまえを付ける (put) name on his chest.
本にタイトルを付ける (give) a title to a book.
He 'named' his son Ken.
彼はむすこにケンと'名付けた'。(concise)
彼はむすこにケンという名前を付けた。
He gave his son a name "Ken".
(direct translation. in English it's roundabout but in Japanese natural.)

"Who gave you your name?" 「名付け親は誰ですか?」
This is a more natural expression in Japanese.
