A road is a physical 'object'. It is a paved surface, usually covered in asphalt or similar material, which vehicles drive along.
A route is not a physical thing. If you plan a route to get to a certain place, you decide what direction you will drive in and what points you will pass on the way, for example.

A road is the physical thing you are travelling along - the street. A route is the path you take to reach the destination. This is also part of a phrase borrowed from French: "en route" which means "on the way."
