People may prefer to buy an otherwise identical shoe when the production process respects certain worker rights.

'otherwise' is something like 'in every sense other than the one I just mentioned'.
so 'otherwise identical' is identical apart from whatever was said before that. Without the context, that's not clear.
Perhaps - it's identical other than the price?
