It is almost always used as a compliment, to mean that the features of your face stand out in an attractive way. I think nowadays it most commonly refers to having a strong cheek and jaw structure, but it can really apply to any features. Of course, the answer to your second question is more of an opinion, but I think most people would say models are anywhere between an 8 and a 10 out of 10 usually.
