This is a table from a textbook, but I think the irregular verb changes it provides are all in the past tense, not the past participle. So why is the English name for the past participle written at the very front as the title? Is there a printing mistake in the book?

I agree with you. It's confusing. All of the words have irregular past participles, but it would be better if there were three columns with the base, past simple and past participle forms. Some of the words in the second list are the past participle (for example "bought"), but some are not ( for example "chose")... perhaps the idea is that it's an exercise where the student adds a third column with the past participles?

yes, there is a mistake here
we can judge it from
go-went-gone which is here given as go-went
but must be go-gone
lame book, burn it
