It depends who is greeting you.
In written communication, 'dear' is a formal way to address someone. It can be followed by their given name (Dear John) or their surname (Dear Mr Smith). It's just a formal and polite way to address someone you don't know really well in writing.
In speech, 'dear' is affectionate. It is a term of endearment. So close friends might address you in this way. It would be followed by your given name. I think this is quite rarely used though.
