This is a kinda difficult question for me because my English aren't good but briefly, Do you always raise the tone on the last syllable? -> Yes, we do. For example, "까" or "요" are typical syllables added to the last word when making a question and should always be raised at the end of the sentence. Can you raise it on other words/syllables to stress other things? -> Yes,it is possible. For example) "How did you make this bread?" In english, like above example, "how" and "bread" are the words stressed because it is important information to listener. Interrogative pronouns, as you know, such as "how"/"when"/"what"/"where"/"who"/"why" are generally stressed in Korean as in English but not always I think. (stressed) "이 빵을 (어떻게) 만들었나(요)?" "(어떻게) 이 빵을 만들었나(요)?" If you want to emphasiz on an object in a sentence, you can do that. For example, "(Why) on earth did you put (Dwight) in charge of the money?" "도대체 (왜) (Dwight)에게 돈을 맡긴거야?"
