"Sleep" is the main verb, meaning the state of rest you experience at night in bed.
To "fall asleep" means to enter into that state of sleep! If you are tired and your eyes begin closing by themselves, you could say "I'm falling asleep, I need to find a bed".
Here is an example that uses both verbs, to show the difference:
"Every night I fall asleep at 10 o'clock. I sleep until 8 o'clock in the morning."
In the example, "I fall asleep" means that I will begin to sleep, and "I sleep" simply means I am in that state of sleeping.

"sleep" is just 睡觉 and it can be a noun or a verb.
I drank too much coffee and now I can't sleep. (verb)
I slept better after I got a new pillow. (verb)
I haven't been getting enough sleep lately. (noun)
The food you eat can affect the quality of your sleep. (noun)
"asleep" (not sleep) is an adjective to describe someone who is sleeping.
Be quiet! The baby's asleep.
Don't call me after midnight. I'll be asleep.
"fall asleep" means "begin to sleep" or "become asleep"
I went to bed at 11:00 but I didn't fall asleep until 12:00.
I fell asleep on the bus and misseds my stop.
Have you ever fallen asleep in class?
Notice that "fallen asleep" is the past participle form of "fall asleep." fall - fell - fallen

i had fallen asleep or he had fallen asleep -past tense
i fall asleep -mostly personal present tense
i go to sleep or he is going to sleep -can be past or present tense but is used alot

sleep = s/he went to bed to sleep (s/he was planning to sleep)
fall asleep = s/he slept without planning to sleep ( slept unintentionally )
fallen asleep = same as (fall asleep) but in past participle
(fall - fell - fallen)
