As Holland says, it is a faint, not quite seen, hint of a smile. Something indistinct. It is more something one sees in writing, perhaps, than in real life.

A faint possibility of something.
If you say:
"He looked at me with a ghost of a smile."
It means he is unhappy but something occurs which causes them to smile just a little bit.
