Not really. You can be sick (e.g. have a stomach ache) without fever.

La diferencia radica en que al decir: "Estoy enfermo", te estás refiriendo a que posees alguna enfermedad (disease). En cambio, al decir "tengo fiebre", estás especificando que tienes fiebre (fever). Saludos

Estoy enfermo is a general, 'I'm ill/sick/not well'. Tengo fiebre is 'I have a fever', which not everyone who is ill will have.
