This is idiom. This mean to stay very strong on the views and to be conservative in thoughts and words. Anyway, English isn't my native language. Maybe natives will add something.

I would use this expression to describe someone's behavior e.g. My grandparents are very set in their ways, they like to do the same things at the same time every day. Someone who is set in their ways doesn't like change and it is usually used to describe older people

It is almost the same as STUBBORN. It is a phrasal verb, which means you add something to a verb and that changes its meaning. "Set" means "fixed", so when cement hardens, it sets. That's why to be "set" in one's ways means to be difficult to change.
