I think that you forgot the most important thing of them all. Learn how to pronounce the words correctly. If you don't know or are not sure how to pronounce a word , search for it on the internet or ask a native speaker. I believe that most people who speak english so bad is because they are not sure how a word really sounds so they are very insecure when they speak.

Proper intonation and pronuciation ought to be the sticking point to have yourself speak as good as a native speaker but the issue is how to pronunce well in a long statement? I consider the only solution to the problem is "to make your speaking as natural as knee jerk (purely tendon reflex without thinking)'.

Therefore, you do have to acquire profound knowledge of English which is native speakers' natural response without thinking but you had better practice as often as possible to keep it fresh given that language learning can be seen as a sport in a way. That means if you are lacking of practice, you are hardly able to speak and listen well.
