Assess, in nature, has the meaning of a judgement. so by assessment you might get more closer to a judgement about something than evaluation does.
whereas, to evaluate , in nature, possesses the meaning of an opinion.
In general, by assessing we analyze, calculate or estimate something, therefor we make a judgement. whereas by evaluating, we look and check the situation to make an opinion .
It's something like " an accident "
when we look around the area, we try to find a solution by thinking carefully .[this is evaluation ]
while assessing the situation would be closer to simply looking and analyzing the situation and making a judgement.

In teaching, assessing a student is obtaining information about that student's performance and ability. Evaluation is comparing the student's performance against the class average or standard levels.
In science, an assessment is performed more at the beginning of the project to note all the initial data and conditions, while an evaluation is done at the end, to note the change in the data and make judgements.
In normal everyday use, however, assessment and evaluation may be interchanged and frequently are.
