None of those are proper English. "You could help me?" would be used in response to someone offering help if you aren't sure they offered help the first time. However, you would use "Could you help me?" if you want to ask someone for help. "Can you help me?" is also correct.

"Can you help me?" or "Could you help me?" are both correct. Never use the preposition "to" after a modal such as "can", "could", "may", "might", "will", "would", "should", etc. The only exceptions are "ought to" and "have to".

A simple tip: "can" and "could" are never followed by "to".
