There are several ways to write them, depending on context and formality. You can choose to write out the whole date, abbreviate the months, abbreviate the year, and even the day. It's also important to remember that in America we (usually) read the date left to right as month, day, year. Although sometimes, more formally, we will write it year, month, day - i.e. 20040509. You will most likely never have to write it this way.
For example:
May 09, 2004 = May 9, 2004 = 05/09/04 = 5/9/04 (very informal, but often all that is required)
December 08, 2008 = Dec. 8, 2008 = 12/08/2008 = 12/08/08 = 12/8/08
January 15, 2001 = Jan. 15, 2001 = 01/15/2001 = 01/15/01 = 1/15/01

I read these dates as day-month-year, so:
The ninth of May...
The eighth of December...
The fifteenth of January...
I'd still write the year with numerals.
