Take care is like saying goodbye or farewell. Be careful is like a warning, to stay away from trouble or danger.

Hi . One says "take care "when saying goodbye. It simply means take care of yourself.
Whereas "be careful" is seen as a warning. eg. "Please be careful when walking down the slippery path"

As Chris said.
"Take care" is very general. It means "Take care of yourself. Go slow, relax, enjoy life, but stay safe, stay healthy, and don't take unnecessary risks."
"Be careful" is usually said when there is an imminent, specific threat.
Wife: What was that noise downstairs?
Husband: I'll go check.
Wife: Be careful! (there could be a burglar downstairs with a knife or gun)
