what does "drive" mean in " drive the conclusion to action plans"?
Does the sentence mean "convert the content of conclusion into action plans" or " move from conclusion into action plans (so the conlusion and the action plans are not so relevant in this sentence)”?

I think we really need the rest of the sentence to understand this. To me, it sounds like the action plans are ongoing, and they need to be concluded, with "drive" meaning push, encourage, speed up.
For example, inventing a first half to the sentence: "The company hired an action plan manager to drive the conclusion to action plans" - in which the manager's role is to get that to happen sooner than it otherwise would.

I read it as saying that they want no lag between conclusion and action. The conclusion must cause action. It is not enough to know what needs to change (conclusion) there needs to be an answerof what to do to change it (action). The meeting is expected to be the end of one problem and the beginning action to solve it.
