1. 时刻表 and 计划表 are the most commonly used ones. 时刻表 is used in case of expressing specific times. For example: train's schedule：火车时刻表。 计划表 is more like a plan. For example, my schedule for this week:我这周的计划表。

日常表 and程序表 are rarely used in daily life.

And 日程表 is another commonly used one. It can be generally used in many cases including specific times or plans.

2.3.:Technically, a playground in a park is 游乐场. An amusement park is 游乐园。But sometimes they are interchangeable  given the context. But without context, most people will think 游乐场 to be a playground in a park and  游乐园  to be an amusement park.
