For me it's like the difference between he has a few friends and he has few friends Usually, personally, I don't say 他的朋友很少，for it's a little bit offensive to me, like i'm judging him

I think translation it depends on the context. Both are referring he has not much friends. But when I usually say 他很少朋友，他没有什么朋友。sometimes in different regions we would speak differently. But universal we say like bean said. 他的朋友不多，他很少朋友。are natural response.

他(的)朋友很少 or 他（的）朋友不多 他有很少朋友 this is not natural。
