您贵姓？ nín guì xìng?
★★★ （1）This sentence is often used in formal occasions or for addressing elders.
There are four kinds of answers:
A: 我姓刘。wǒ xìng Líu.
B: 免贵姓刘。miǎn guì xìng Líu.
C: 我姓刘，叫刘德华。 wǒ xìng Líu, jiào Líu Déhuá.
D: 免贵姓刘，叫刘德华。 miǎn guì xìng Líu, jiào Líu Déhuá.
★★ （2）For informal occasion, especially children, people use “ ‘ni jiao shenme mingzi?你叫什么名字？’ or ‘ni jiao shenme?你叫什么？’ ” instead of “nín guì xìng?”.
The answer is “ wǒ jiào Sòng Lìli.” or Just say your name, “Sòng Lìli.”
★ （3）Some of people may will say “nín zěnme chēnghū?您怎么称呼？” or “zěnme chēnghū nín ?怎么称呼您？” .
This sentence means that he would like to know your name.
Answers: A and C in (1) are OK.

Hi, Mark. No people speak like that. You speak:你叫什么(名字)？or "您贵姓？" or "(您/你)怎么称呼?"
If they give you English name because you are a foreigner, just spaek"中文名字呢？" It's ok.

Hi nadia,if two students meet and they have nearly age and identity,"你叫什么？"is ok for this scene.
If you want to show your respect "您贵姓?" is a better choice.
