2) Do you say "A creepy pretty boy" or " A creepy, pretty boy"... "Success is about quiet little things" or "Quiet, little things"? And finally, "a cute chubby small dog" or "A cute, chubby, small dog"? Neither? "A cute, chubby small dog"?

I think most common, when talking about shoes and sizes, would be "These are a size 11" or "These are a size 11." "These are 11" or "These are a/an 11" sounds a bit unnatural, even if it's clear that shoes are the subject.
