I assume you're trying to ask if people are afraid of lizards in general, and for this we use a general, "lizards" without a capital. So the sentence would be, "/Are you or anybody from your family or friends afraid of lizards?" "If yes then why?" is fine. We may put in a comma, "If yes, then why?" I am not afraid of lizards, though it's really freaky if you ever accidentally pull their tails off and they keep twitching.
