If you are 'perseverant', it means that you can keep doing your work with determination, even if you face many problems on the way. We usually talk about perseverance when we are discussing challenges and problems in life.
if you are 'patient', it means that you have the ability to accept delay, suffering, or annoyance without complaining or becoming angry.

Patient entails not losing your calm or determination on something regardless of how stressful, hopeless or annoying a situation may seem.
Perseverant would entail working hard towards a goal despite all difficulties, obstacles or setbacks one may have.
I suppose “patient” would be more fitting to describe a calm and considerate attitude towards someone, whereas “perseverant” would describe a pattern of actions that show someone not giving up.
