I agree with Charlie ; the term ‘general date’ doesn’t have any meaning, it’s a data input designation that shouldn’t appear on the form when printed.

Well I'm guessing this comes from some kind of template, and that in the place marked "general date", you would simply fill in the date on which you filled in the form, or completed the document... I haven't seen this before...
