Well said.

Also, in my opinion, a high-intermediate level (B2) is needed for conversation. Beginners simply don't have the grammar or the vocabulary to say much (that an unpaid listener wants to hear).

Language EXCHANGE should provide the same benefits to both. If the agreed rules are clear and both partners are at the same level, it can be both educational and entertaining.
I always look forward to my weekly session with my Skype-partner and prepare questions or a brief report/commentary etc. to test words I'm not sure about their adequate usage.
We split the session in two parts and try to be disciplined. Sometimes, much to my regret, my time runs off too fast. Then it can happen that I forget to switch to my first language. At such moments, I feel comfortable and relaxed (while I speak English). That's why I always put a big clock on my desk!
Not to forget: For me, it is very important to save the "new words" and apply them as often as possible.
Gratitude? Yes! Maybe I should mention it more frequently!!! Yes!!!

I think you have to add

Fear; fear of looking a fool, fear of failure, fear of strangers

Confusion; I've had people missunderstand what I'm saying in my mother tongue, taking offence because they don't understand. A good example is the phrase "I'm sorry". In English that can mean "what a pity, it was my fault" or "what a pity" if the other language user is only aware of the first translation they can get confused.
